245D Waivered Services:

  • Beacon home care is licensed provider of 245D basic services including Individualized Home Supports Without Training (IHS), Night Supervision, Homemaking, Respite and Individual Community Living Supports (ICLS). For each service, you select a trusted worker who best suits your needs and maintain control of how your workers provide services by scheduling hours and providing them direction on your specific day to day support needs.
  • Our staff partners with you and your case manager to initiate services, develop and implement your support plan and work to ensure you are receiving a high quality of care. 245D Basic Services:

  • 245D Basic Services refer to essential, person-centered support provided to individuals with disabilities, ensuring their safety, well-being, and independence in their daily lives. These services are designed to meet the unique needs of each individual by offering assistance with activities of daily living (ADLs), health monitoring, and emotional support. By focusing on promoting independence and dignity, 245D Basic Services help individuals maintain a higher quality of life in their preferred environment, whether at home or in the community.


245D Intensive Services:

  • 245D Intensive Services provide a higher level of support for individuals with more complex care needs, ensuring they receive personalized and comprehensive assistance to live safely and comfortably. These services are designed to address significant physical, medical, behavioral, or emotional challenges that may require specialized care and monitoring. By offering a structured, person-centered approach, 245D Intensive Services empower individuals to maintain their dignity, independence, and quality of life while meeting their advanced care needs.


Basic Support Services

These are services that provide the level of assistance, supervision and care that is necessary to ensure the health and safety of the person. 

  • Individual community living supports (ILCS): For people who need reminders, cues intermittent/moderate supervision or physical assistance to remain in their own homes.
  • 24-hour emergency assistance: On-call counseling and problem-solving and/or immediate response for assistance at a person’s home due to a health/personal emergency.
  • Adult Companion: Non-medical care, supervision and socialization to a person age 18 or older.
  • Homemaker: Eligible services range from light household cleaning to household cleaning with incidental assistance with home management and/or activities of daily living.
  • Night supervision: Provides overnight assistance and monitoring by an awake staff in the person’s home.
  • Respite care: Short-term care due to the absence or need for relief of the family member(s) or primary caregiver normally providing the care.
  • Individualized home supports without training: Services for adults or children when they need support, assistance and supervision in at least one of the community living service categories when living in their own home or family home. Community living service categories include community participation; health, safety and wellness; household management; adaptive skills.


Intensive Support Services

  • Semi-independent living skills: Services needed by an adult with a developmental disability or related condition(s) to live successfully in the community.
  • Individualized home supports: Provide the support in the areas of community participation, health, safety & wellness, household management, and adaptive skills.
  • Individualized home supports with training: Provide the support and training in the areas of community participation, health, safety & wellness, household management, and adaptive skills. The training is designed to help the person develop the skills they need to live independently.
  • Individualized home supports with family training: Provide the support and training to both the person with a disability and their family. The training is designed to help the family learn how to support the person in their home and community.
